Home Health Speech Therapist
Part-Time (24 hrs/wk) Day Shift
In this role, you will provide independent, age appropriate speech therapy evaluation and treatment to patients in their place of residence. You will teach the patient/family and caregivers preventative and rehabilitative procedures. In addition, you will develop and coordinate the plan of care and provide case management.
At least two years of experience in speech therapy
A graduate of an accredited speech therapy curriculum with a master’s degree
A current license to practice as a Speech Therapist in California by employment start date
American Heart Association approved BLS certification
A current and valid California Driver’s licensed
National Provider Identifier (NPI) or the ability to obtain one prior to employment start date
Clinically skilled to detect early warning signs of medical complications including mentation
Good interpersonal skills
The ability to differentiate emergent versus non-emergent patient care status
The ability to look at patient holistically, not just body part of functional ability
The commitment to service orientation
Effective written and oral communication skills (in English)
The ability to work in a Labor/Management Partnership environment
Preferred Qualifications Include:
Experience in an automated clinical information system preferred
Computer literate preferred
Knowledge of Medicare conditions of participation and OASIS preferred
Home Health Experience preferred
